CARE COORDINATOR

Physicians CareConnection, the largest coordinator of voluntary health care services in central Ohio and locally recognized for its unique and extraordinary model of care coordination, is actively seeking a bilingual Spanish speaking Care Coordinator to join our dynamic team.

The primary responsibilities involve coordinating medical care for low income, uninsured people. Coordinator will also be responsible for building and maintaining relationships with volunteers in the local health care community.

Position requires: Associate degree in healthcare or social services related field preferred or equivalent community related experience; college degree preferred. Good Typing Skills; Excellent Communication and Time Management Skills; Software Proficiency: Word, Excel, and Outlook; Mature, professional and positive attitude; Interest in working with low-income and diverse populations.
