The PFC/VCN is a vital part of the health care safety-net. It connects low-income, uninsured residents with medical homes and improves access to specialty care. Over 30,000 vulnerable people have been helped by coordinating and connecting health care safety net services. For every $1 we receive in funding, we coordinate $5 in medical care services, which demonstrates a 500% return on investment.
